DEVELOPMENTAL EVALUATIONS

Detecting problems early on is essential to your child’s health. We use multiple tools/screeners to aid in this process. During well child visits we ask that you fill out different forms depending on age. You can access forms on CHADIS.com, track important milestones and parenting tips with the HEALOW APP, and sign up for Patient Portal to stay involved with your child’s health.

This form will need to be completed before each well check for children above the age of seven. To cut down on wait time, please have this form completed and bring with you at your child’s next well check.
