For those who may have concerns that their child may have ADD/ADHD, anxiety, depression or mood disorders, we ask that you fill out an initial parent and teacher form. When completed please bring by office and allow us time to review them. Once reviewed, we set up appointment as soon as possible.
We provide a thorough initial evaluation for suspected ADHD. In this evaluation we will also screen for other behavioral issues. Please expect to be here for at least an hour.
If your child is evaluated and it is determined he/she has ADD/ADHD, it may be determined that a stimulant medication will be prescribed. These medications are considered a schedule II-controlled medication and are monitored by the DEA (The Drug Enforcement Administration). Rainbow Kids Clinic believes in treating every patient with a plan that is tailored to that individual and in the safest way possible. This includes monitoring, collaborating, and sharing with those that care for the child to bring out the best all around results. Below is our Controlled Substance Contract for ADHD Therapy that must be signed and updated at least once annually. To save time you may read/print this out to bring to your child’s appointment.
