If you have decided to have this elective procedure performed in our office, you may review our consent form to save time. This will allow you time to think of questions or concerns you may have to ask us. Write down any pertinent information and questions to bring with you. This will ensure you remember what to discuss with us. We go over everything and make sure all questions are answered.
